Try uninstalling Creative Cloud Desktop using the uninstaller, then reinstall from the CC site, reopen Photoshop (I had to do it twice for some reason). It worked for me after trying all sorts of stuff with Extension Manager that didn't work. I've done exactly the same thing on two Macs and both times this brought the Extensions menu back.
